Access My Records
You have the right to see your Medical Records, dated from November 1991, subject to any limitation in law.
We cannot provide you with access to records of any family members or friends without their written consent.
If you require access to your notes, you must write to us requesting this. If there is any reason why we cannot process your request we will inform you as soon as possible. When permission is granted, we will invite you to the surgery to review your records. A member of staff will be present while you review your notes.
